COMMUNITY HEALTH SYSTEMS
Many communities facing healthcare delivery system changes are struggling just to maintain their current health care services.  We offer a way to facilitate community evaluation, discussion and decision-making processes addressing access to primary healthcare.  In 2003-2004 we expanded our capacity for conducting community assessment and community development activities and initiated 61 community health system support projects.                                               Back

Community Health Systems Highlights

Prairie AHEC has served as a lead agency for a leadership committee for the 100% Access/0 Disparities initiative in Denton County. Among the successes are infant mortality programs for prenatal care and sonogram testing.  Retreats, monthly meetings, and community assessments are part of the project to date. Action items for the future include grant applications, foundation requests, and county assistance presentations. This partnership has included all of the for-profit hospitals in the county to address indigent and uninsured healthcare.

Lake Country AHEC was the contractor responsible for creation of The Texas Toolbox for Community Health Development, an online resource to support community health development.  It contains hands-on resources for developing community recruitment and retention plans, and informational materials from public and private agencies to support health systems analysis and development. The online version is the third edition of the Toolbox. It is a project funded by The Robert Wood Johnson Foundation through the East Texas Rural Access Program (ETRAP), a part of The Southern Rural Access Program (SRAP)

Brazos AHEC assisted two hospital districts in a merger that allowed both districts to maintain quality healthcare services.  The merger resulted in building one hospitals together.   We facilitated town hall meetings to inform citizens of the proposed changes.
